Review of deadly Ingram Square fire reveals possible flaws in firefighter procedure

SAN ANTONIO -- A state investigation is complete and details are out on the fire that killed San Antonio Firefighter Scott Deem last year.

The state will publish that report officially Friday. It shows a failure to use a hose line or search line to follow out of a fire.

We spoke to the San Antonio Professional Firefighters Association and they released the following statement:

This peer review is something we have been awaiting. We will continue to honor our brother Firefighter Scott Deem as well as all the brave firefighters that fought this fire.
We will evaluate everything and continue to train and keep our Fire Department prepared and ready for any and all situations we encounter.

Robert Vasquez, who was injured the night of the fire, has returned to work. Brad Phipps, the other firefighter injured has not returned to duty.

Scott Deem's wife is out of town and is expected to make a comment when she returns.
